This is where you can sign-up to be listed in the Provider Directory.
 
Any provider in the Bay Area, maintaining a current license in good standing with Communtiy Care Licensing, is welcome to submit a request and be listed in the Provider Directory.
 Providers pay no fee to be listed in the directory. 
 
You may submit your request and information for your listing in one of three ways.
1. If you already have a website, you can request that we simply link to your site;
2. If you do not already have a website, you may request that you be listed with
just your name and contact information; or
 
3. If you do not already have a website, you may request that we create a one page listing for you. 
There is a $25 fee for the development of each webpage.
